You will get a summary printout including…
Record of last relevant consultation with GP
List of current medical problems & medications.

If any further correspondence is required from GP and GP agrees to this, there may be a further charge which will be detailed before we go ahead with any work. This may also increase the wait time.

Fees - First request for a printout/records is is free but £10.00 for additional requests for the same information.

Our practice has decided to outsource our medical reporting work to an NHS Digital
accredited company called MediData.
MediData will be processing your medical report via eMR and providing online access via
their secure encrypted portal.
