To register for our access to your medical record through the NHS App, you will need to download the NHS app and register. You will also need to complete this form and then visit the practice, bringing with you a form of identification which ideally should include your photograph.

We will then process your request within 28 days.

Please highlight to us if you have any concerns about being coerced into giving access to your records to a friend or family member as we may have to give limited access only.
